Lutheran Life Villages' Pine Valley campus is seeking qualified applicants to join the already strong nursing team at the premier campus located on the north side of Fort Wayne. This MDS coordinator will be working in tangent with existing MDS nurses and will be responsible for directing and coordinating assessments, planning, implementation, and evaluation of planned resident care. Ensures continued periodic documentation pertaining to special resident populations as required by State or Federal regulations.

Job Qualifications


  • RN (Registered Nurse) preferred with degree from an accredited state approved school of nursing with a current license to practice in the State of Indiana.
  • LPN (Licensed Practical Nurse) with desired experience may be considered.
  • ICD-10 coding experience preferred.


  • Two (2) years experience in geriatric or long-term care nursing.
  • One (1) year experience in supervisory role, preferred.

3.Required Knowledge, Skills, and Abilities:

  • Strong verbal, organizational, interpersonal and assessment skills.
  • Ability to organize and prioritize work to meet deadlines.
  • Understanding of the concepts of aging and the needs and concerns of the elderly.
